Brompheniramine, Dextromethorphan, Phenylephrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Beta-methylphenethylamine (bmpea) interacts with.

Theoretically, BMPEA might increase the levels and clinical effects of CYP2D6 substrates.
In vitro research shows that BMPEA strongly inhibits CYP2D6.

Here's the gist with Dimetapp DM. Two pieces of this combo, the dextromethorphan (the cough suppressant) and the brompheniramine (the antihistamine), are partly cleared by a liver enzyme called CYP2D6. In lab studies, BMPEA appears to block that enzyme. If that happens in the body, these ingredients could build up and hit harder than expected, meaning more drowsiness, dizziness, dry mouth, and with high dextromethorphan levels, possible confusion or feeling spacey.

On top of that, BMPEA is an amphetamine-like stimulant, and the phenylephrine in this product is also a stimulant that bumps up blood pressure and heart rate, so stacking them is a real concern. This interaction is theoretical (based on lab data, not people), but BMPEA is best avoided anyway. Please check with your pharmacist before combining them.

Here's the piece that matters most in your Dimetapp DM: the phenylephrine. That ingredient is a decongestant that already nudges your blood vessels to tighten, which can bump up your blood pressure and heart rate a bit. BMPEA is a hidden amphetamine-like stimulant, so stacking it on top of phenylephrine could, in theory, push those numbers higher together.

The realistic worry is feeling jittery, a racing or pounding heartbeat, headache, or a spike in blood pressure. To be fair, this is a theoretical concern based on how these compounds behave, not proven cases. Still, BMPEA is an unapproved, poorly studied ingredient I'd steer clear of anyway. If you take anything containing it, please check with your pharmacist or doctor first, especially if you have any heart or blood pressure history.

Beta-methylphenethylamine (BMPEA) is a synthetic stimulant chemically similar to amphetamine that has been found in some weight-loss and sports supplements, often without being clearly listed on the label.
