Doravirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Beta-methylphenethylamine (bmpea) interacts with.

Theoretically, BMPEA might increase the levels and clinical effects of CYP3A4 substrates.

Here's the short version: your HIV medicine doravirine (Pifeltro) is cleared from your body mainly by a liver enzyme called CYP3A4. BMPEA, a stimulant found in some weight-loss and sports supplements, has been shown in lab dishes to slow that same enzyme down by a little (around a third). If it does the same thing in people, doravirine could build up a bit higher than usual, which might mean more side effects like nausea, headache, dizziness, or trouble sleeping.

I want to be honest about how soft this evidence is. It's theoretical, based only on test-tube data, not real patients. Still, BMPEA isn't an approved or well-studied ingredient and carries its own heart risks, so I'd skip it. Please check with your pharmacist or doctor before mixing the two.

What Is Doravirine Used For?

Brand name: Pifeltro

Pifeltro (doravirine) is used together with other antiretroviral medications to treat HIV-1 infection in adults and in pediatric patients weighing at least 35 kg (about 77 lbs). It is approved for people who have never been on HIV treatment before, as well as for people who are already virologically suppressed — meaning their HIV is currently undetectable — and want to switch to a new stable regimen.

Pifeltro is not used alone; it must always be part of a combination HIV regimen. Patients switching to a Pifeltro-based regimen must have no history of treatment failure and no known resistance to doravirine.
