Dextromethorphan, Guaifenesin, Pseudoephedrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bitter Orange interacts with.

Bitter orange might increase levels of drugs metabolized by CYP3A4.
Small clinical studies suggest that single or multiple doses of freshly squeezed bitter orange juice 200-240 mL can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. However, the extent of the effect of bitter orange on CYP3A4-mediated drug interactions is unknown. Some evidence suggests that bitter orange selectively inhibits intestinal CYP3A4, but not hepatic CYP3A4. Its effect on P-glycoprotein, which strongly overlaps with CYP3A4 interactions, is unclear. One small clinical study shows that drinking 8 ounces of freshly squeezed bitter orange juice has no effect on cyclosporine, which seems to be more dependent on hepatic CYP3A4 and P-glycoprotein than intestinal CYP3A4.

Here's the thing worth knowing about this combo. Bitter orange can mildly block CYP3A4, an enzyme in your gut that helps break down certain medicines. Of the three ingredients in your cough and cold product, dextromethorphan is the one partly handled by this pathway, so its levels could nudge upward. None of these are prodrugs, so more drug simply means a stronger effect, things like drowsiness, dizziness, or feeling foggy.

There's a separate concern I want to flag. Both bitter orange (synephrine) and the pseudoephedrine in this product are stimulants, so stacking them may raise your heart rate and blood pressure more than either alone. The evidence here is limited and mostly from juice studies, but if you have heart issues or high blood pressure, please check with your pharmacist or doctor first.

Here's the thing to watch with this combo: your cold and cough product already contains pseudoephedrine, which is itself a stimulant decongestant. Bitter orange brings its own stimulant punch from synephrine, a compound that acts a lot like the body's adrenaline. Stack the two and you're essentially doubling up on stimulants.

That can nudge your heart rate and blood pressure higher, and may leave you feeling jittery, anxious, or like your heart is racing or pounding. Some people notice trouble sleeping or headaches too. The concern is mostly theoretical, but it's a reasonable one, especially if you have high blood pressure or any heart trouble. Please check with your pharmacist or doctor before pairing them, rather than changing anything on your own.

One small clinical study shows that bitter orange juice increases dextromethorphan levels, likely through cytochrome P450 3A4 (CYP3A4) inhibition. Theoretically, bitter orange might increase the risk for dextromethorphan-related adverse effects.

Here's the thing to know about this combo product: it contains dextromethorphan (the cough suppressant), and bitter orange can slow down one of the liver pathways (CYP3A4) that helps clear it. A small study found bitter orange juice raised dextromethorphan levels, so in theory the dextromethorphan could hang around a bit longer and feel stronger. That might mean more drowsiness, dizziness, or a foggy, lightheaded feeling.

Here's the gist for this cough and cold combo. One part of it, dextromethorphan, is cleared by a liver enzyme called CYP2D6. A compound in bitter orange (octopamine) has been shown in test-tube studies to weakly block that enzyme. In theory, that could let dextromethorphan build up a bit, nudging up side effects like drowsiness, dizziness, or feeling foggy. This effect has only been seen in the lab, never reported in real people, so I wouldn't lose sleep over it.

My bigger concern is overlap. Bitter orange contains synephrine, a stimulant, and this product already has pseudoephedrine, another stimulant. Stacked together they can raise your heart rate and blood pressure or leave you jittery. Please run this combo by your pharmacist or doctor first.
