CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Dextromethorphan, Guaifenesin, Psuedoephedrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bitter Orange interacts with.
Small clinical studies suggest that single or multiple doses of freshly squeezed bitter orange juice 200-240 mL can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. However, the extent of the effect of bitter orange on CYP3A4-mediated drug interactions is unknown. Some evidence suggests that bitter orange selectively inhibits intestinal CYP3A4, but not hepatic CYP3A4. Its effect on P-glycoprotein, which strongly overlaps with CYP3A4 interactions, is unclear. One small clinical study shows that drinking 8 ounces of freshly squeezed bitter orange juice has no effect on cyclosporine, which seems to be more dependent on hepatic CYP3A4 and P-glycoprotein than intestinal CYP3A4.

Here's the gist for your Aquatab C, which combines dextromethorphan (cough), guaifenesin (loosens mucus), and pseudoephedrine (a decongestant). Bitter orange may block CYP3A4 in your gut, and dextromethorphan is partly broken down by that enzyme. In theory, that could let a bit more dextromethorphan build up, which might mean more drowsiness, dizziness, or feeling foggy.
What I'd watch more closely is the overlap of stimulants. Bitter orange's synephrine and the pseudoephedrine in this product can both nudge up heart rate and blood pressure. Together they may leave you feeling jittery, restless, or with a racing heart. The evidence here is limited, but if you have high blood pressure or heart issues, please check with your pharmacist or doctor first.

Here's the thing to watch with this combo. Your Aquatab C already contains pseudoephedrine, which is itself a stimulant decongestant. Bitter orange supplements contain synephrine, another mild stimulant that's chemically a cousin of ephedrine. Stacking two stimulants together can add up.
The concern is mostly about your heart and blood pressure. Together they could nudge your heart rate up, raise your blood pressure, or leave you feeling jittery, anxious, or unable to sleep. This is a theoretical, moderate concern, not a guarantee, but it matters more if you have high blood pressure or any heart trouble. The cough and mucus ingredients (dextromethorphan and guaifenesin) aren't the issue here. Before combining them, check with your pharmacist or doctor.

DEXTROMETHORPHAN (Robitussin DM, others)
3 of 4One small clinical study shows that bitter orange juice increases dextromethorphan levels, likely through cytochrome P450 3A4 (CYP3A4) inhibition. Theoretically, bitter orange might increase the risk for dextromethorphan-related adverse effects.

Here's the deal with this combo. Your Aquatab C has three pieces, but the part that matters here is the dextromethorphan (the cough suppressant). A small study found that bitter orange can slow down a liver enzyme called CYP3A4, which is one of the pathways that clears dextromethorphan out of your body. When that pathway is partly blocked, dextromethorphan can build up a bit higher than usual.
In real terms, that could mean more of its side effects, like drowsiness, dizziness, or feeling foggy. On top of that, bitter orange is a stimulant and the pseudoephedrine in this product is too, so together they could nudge your heart rate and blood pressure up. The evidence is fairly limited, so don't panic, but it's worth running past your pharmacist or doctor before combining them.

In vitro research shows that octopamine, a constituent of bitter orange, weakly inhibits CYP2D6 enzymes. This effect has not been reported in humans.

Here's the thing to keep in mind with this combo. One ingredient in your Aquatab C, the cough suppressant dextromethorphan, is broken down by a liver enzyme called CYP2D6. A compound in bitter orange (octopamine) has been shown in test-tube studies to weakly block that same enzyme. If that happened in your body, dextromethorphan could build up a bit and lead to more drowsiness, dizziness, or a foggy feeling.
But I want to be straight with you: this is only a theoretical concern based on lab research, never actually seen in real people, and it's rated as minor. On top of that, bitter orange is a stimulant and your product already contains pseudoephedrine, another stimulant, so together they could nudge your heart rate and blood pressure up.
