Dextromethorphan HBr, Phenylephrine HCl falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bitter Orange interacts with.

Bitter orange might increase levels of drugs metabolized by CYP3A4.
Small clinical studies suggest that single or multiple doses of freshly squeezed bitter orange juice 200-240 mL can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. However, the extent of the effect of bitter orange on CYP3A4-mediated drug interactions is unknown. Some evidence suggests that bitter orange selectively inhibits intestinal CYP3A4, but not hepatic CYP3A4. Its effect on P-glycoprotein, which strongly overlaps with CYP3A4 interactions, is unclear. One small clinical study shows that drinking 8 ounces of freshly squeezed bitter orange juice has no effect on cyclosporine, which seems to be more dependent on hepatic CYP3A4 and P-glycoprotein than intestinal CYP3A4.

Here's the situation with this cough and congestion combo. Bitter orange can mildly block an enzyme in your gut called CYP3A4, which helps break down the dextromethorphan part of this product. If that breakdown slows, more dextromethorphan can hang around, so you might feel more drowsy, dizzy, or lightheaded than usual.

There's a second concern that worries me more. Bitter orange is a stimulant, and the phenylephrine here is also a stimulant that narrows blood vessels. Stacking the two could nudge your heart rate and blood pressure up together. The CYP3A4 evidence is modest and comes from fresh bitter orange juice, not every supplement, but to be safe please run this past your pharmacist or doctor first.

Here's the thing to watch with this combo. Your Vicks Formula 44D contains phenylephrine, a decongestant that works by tightening blood vessels, which can nudge up your blood pressure and heart rate. Bitter orange contains synephrine, a stimulant that does much the same thing. Stack two stimulant-type ingredients together and the effects can add up, so you may notice a faster or pounding heartbeat, jitteriness, headache, or a rise in blood pressure.

This is rated a moderate, possible concern, and the evidence is on the theoretical side rather than proven, so don't panic if you've taken them together once. Still, if you have high blood pressure or any heart trouble, it's worth checking with your pharmacist or doctor before pairing them.

The bitter orange in many weight-loss or energy supplements can slow down one of your liver enzymes (CYP3A4) that helps clear the dextromethorphan cough suppressant in this product. A small study found that bitter orange juice raised dextromethorphan levels, so combining the two could leave more of it in your system than expected. That might mean more drowsiness, dizziness, or feeling foggy and lightheaded.

In vitro research shows that octopamine, a constituent of bitter orange, weakly inhibits CYP2D6 enzymes. This effect has not been reported in humans.

Here's the gist with this combo. The dextromethorphan in your Vicks 44D is cleared by a liver enzyme called CYP2D6. Bitter orange contains a compound (octopamine) that, in lab dishes, weakly slows that same enzyme. In theory, that could let dextromethorphan build up a bit, meaning more drowsiness, dizziness, or a foggy feeling. Just keep in mind this has only been seen in test tubes, not in real people, so it's rated very minor.

I'd worry more about the stimulant side. Both the phenylephrine (a decongestant) and bitter orange's synephrine can nudge up your heart rate and blood pressure. Together they may add up, so if you have any heart issues or high blood pressure, check with your pharmacist before pairing them.
