Here's the thing to know about pairing bitter orange with a product that contains pseudoephedrine (the decongestant part of these combo medicines): both are stimulants. Pseudoephedrine already nudges up your heart rate and blood pressure to shrink swollen nasal passages, and bitter orange's synephrine works in a similar adrenaline-like way. Stacking the two can add up.
So together, you could see more jitteriness, a faster or pounding heartbeat, trouble sleeping, or higher blood pressure than either would cause alone. This is mostly a theoretical concern based on how these compounds behave, not strong head-to-head studies, so don't panic. But if you have heart trouble or high blood pressure, it's worth a quick chat with your pharmacist or doctor before combining them. The guaifenesin part isn't the issue here.
