Conjugated Estrogens, Methyltestosterone falls under ORAL DRUGS — the group Blond Psyllium interacts with.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Conversely, case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: blond psyllium is a soluble fiber that turns into a thick gel in your gut. Because your Premarin with Methyltestosterone is swallowed and absorbed through that same gut, the fiber gel could grab onto a little of the hormones and slightly change how much gets into your body. The honest truth is the science is mixed. In animal studies psyllium actually increased estrogen absorption, while with other drugs it lowered absorption, so we really can't predict the exact direction here.
The good news is this is rated a minor, theoretical concern. To stay on the safe side, take your psyllium about 30 to 60 minutes after your hormone pill, not at the same time. If you notice new spotting, breast tenderness, or your symptoms returning, check in with your pharmacist or doctor.

ETHINYL ESTRADIOL

Concurrent use of blond psyllium with ethinyl estradiol results in a slight increase in the extent of ethinyl estradiol absorption and a slower rate of absorption. However, this is unlikely to be clinically significant.

Here's the short version: when you take a fiber supplement like blond psyllium close to your Premarin with Methyltestosterone, that thick gel it forms in your gut can slightly slow down and gently nudge how much estrogen gets absorbed. The good news is this is based only on lab-type data with ethinyl estradiol, and it's rated as a minor and unlikely concern. For most people, it won't make a noticeable difference in how your medication works.
To keep things simple, many folks just space the two apart, taking your hormone pill about two to four hours away from the psyllium. If you notice anything different, like changes in breakthrough bleeding or symptom control, give your pharmacist or doctor a quick call before changing anything.
