Estradiol, Norethindrone Acetate falls under ORAL DRUGS — the group Blond Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Conversely, case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin.

Here's the simple version: Activella combines an estrogen (estradiol) with a progestin (norethindrone) that your body absorbs through the gut. Because blond psyllium turns into a thick gel in your stomach, it can grab onto pills and change how much of the hormone gets soaked up. The honest truth is the effect goes both ways. Some animal research actually showed psyllium increased estrogen absorption, while in other cases fiber can slow or reduce how a drug is taken in.

This is rated a minor interaction and the evidence is fairly thin, so I don't want you to worry much. The easy fix is timing: take your Activella first, then wait about 30 to 60 minutes before your psyllium with plenty of water. That spacing keeps absorption steady. Theoretically, taking blond psyllium at the same time as ethinyl estradiol might alter levels of estradiol.
Concurrent use of blond psyllium with ethinyl estradiol results in a slight increase in the extent of ethinyl estradiol absorption and a slower rate of absorption. However, this is unlikely to be clinically significant.

Here's the good news if you take Activella (estradiol with norethindrone) and want to add a psyllium fiber supplement: this combination is considered very low concern. The thinking is that psyllium's thick, water-holding gel could grab onto a little of the hormone in your gut and slow how quickly it gets absorbed. Interestingly, the lab data actually suggests it might slightly increase the total amount absorbed, not block it, but either way the change is tiny.

This is only theoretical and comes from test-tube and animal work, so I wouldn't lose sleep over it. To play it safe, simply separate them by a couple of hours and take your fiber with plenty of water. If you have questions about your specific routine, your pharmacist is happy to help.
