Pretomanid falls under IMMUNOSUPPRESSANTS — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might alter the effects of immunosuppressive drugs.
Some in vitro research suggests that Boswellia serrata extracts might inhibit mediators of autoimmune disorders such as leukotrienes and reduce production of antibodies and cell-mediated immunity. However, other in vitro research suggests that, when coupled with calcium ions, boswellic acids containing the keto group have immunostimulant properties within specific cell signaling pathways.

Here's the honest picture: the interaction on file is really about how Boswellia might nudge the immune system, and it's grouped with immunosuppressant drugs. Pretomanid isn't truly an immunosuppressant though. It's an antibacterial you take with bedaquiline and linezolid to fight tough, drug-resistant tuberculosis, so this particular concern doesn't map neatly onto it.

On top of that, the evidence here is very weak, coming from lab and cell studies only, and the lab results even point in opposite directions. So I wouldn't expect Boswellia to meaningfully change how well your TB treatment works or how you tolerate it. Still, TB therapy is serious business and not something to tinker with. Please run any supplement, including Boswellia, past your pharmacist or TB doctor first so they can keep an eye on things.

Boswellia serrata is a tree resin used in traditional medicine, mainly for joint pain and inflammation. Some studies suggest it may help with osteoarthritis symptoms, but the overall evidence is limited and more research is needed. It is generally well tolerated, but you should check with your pharmacist or doctor before using it, especially during pregnancy or breastfeeding.

About Pretomanid

What Is Pretomanid Used For?

Brand name: Pretomanid

Pretomanid Tablet is used only as part of a three-drug regimen — together with bedaquiline and linezolid — to treat adults with pulmonary (lung) tuberculosis that is resistant to multiple antibiotics, including isoniazid, rifamycins, a fluoroquinolone, and a second-line injectable TB drug. It is also used for adults whose TB is resistant to isoniazid and rifampin and who cannot tolerate or are not responding to standard TB therapy.

Pretomanid Tablets are not for drug-sensitive TB, latent (inactive) TB infection, TB outside the lungs, or TB that responds to standard treatment. They should only be used in combination with bedaquiline and linezolid — safety and effectiveness with other drug combinations have not been established.
