simvastatin/ezetimibe falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Here's the gist with Vytorin (simvastatin combined with ezetimibe). The simvastatin part is cleared by a liver enzyme called CYP3A4. In test-tube studies, boswellia mostly seems to slow down that enzyme, which could let simvastatin build up higher than expected. When simvastatin levels climb, the main worry is muscle aches, weakness, or (rarely) more serious muscle breakdown. The ezetimibe portion isn't really affected here.

That said, this is purely theoretical and based on lab data, not real patient reports, so I wouldn't lose sleep over it. Still, since muscle side effects matter, just let your pharmacist or doctor know you're using boswellia so they can keep an eye out and confirm it's fine for you.
