Boswellia Serrata + Zanubrutinib
Zanubrutinib brand name: Brukinsa.
Boswellia Serrata has a moderate interaction with Zanubrutinib. Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.

Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Zanubrutinib
Zanubrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with Brukinsa (zanubrutinib) and Boswellia. Your body relies on a liver enzyme called CYP3A4 to break down zanubrutinib. Boswellia, in lab studies, appears to slow that same enzyme down at most doses. If it does that in real life, zanubrutinib could clear more slowly and build up in your blood, which may mean more side effects like easy bruising or bleeding, lower blood counts, infections, or feeling unusually tired.
I want to be honest about how soft this evidence is. It comes only from test-tube research, not from people, so we don't truly know if it matters at supplement doses. Still, with a serious cancer medicine like this, I wouldn't add Boswellia without first running it by your oncologist or pharmacist.

Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
- AHigh-quality meta-analysis (quantitative systematic review).
- BCase-control study.
- CConsensus.

By severity: 3 major · 119 moderate · 38 minor.
From our pharmacist’s overview on the Zanubrutinib page
“Grapefruit is the biggest supplement risk with zanubrutinib (Brukinsa), a targeted therapy for certain blood cancers, and this list deserves genuine care. Grapefruit blocks the enzyme (CYP3A4) that clears the drug, so levels can climb and side effects with them.”
