Aspirin, Hydrocodone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

This combo product pairs aspirin with hydrocodone, an opioid used for pain. Here's the wrinkle: hydrocodone is actually a prodrug of sorts. Your liver enzymes, including CYP3A4, help break it down and shift it toward its more active form. In lab and animal studies, Bulbine natalensis appears to speed up (induce) CYP3A4. So in theory it could change how much active drug you get and how fast your body clears it, possibly leaving you with weaker or less predictable pain relief.

Honestly, this is only a theoretical concern from test-tube and rodent data, never confirmed in people. I'd be more cautious about Bulbine's own reported liver and kidney effects. Before mixing them, please check with your pharmacist or doctor.

Theoretically, combining Bulbine natalensis leaf with drugs that have anticoagulant or antiplatelet activity might increase the risk of bruising and bleeding.
In vitro research shows that Bulbine natalensis ground leaf extract inhibits platelet aggregation.

This combo product pairs aspirin with hydrocodone, and the part that matters here is the aspirin. Aspirin already thins your blood a bit by keeping platelets from clumping together. In lab (test tube) studies, Bulbine natalensis appears to do something similar. So in theory, stacking the two could add up and make you more prone to easy bruising, nosebleeds, bleeding gums, or a cut that takes longer to stop.

I want to be honest with you: this is a theoretical concern based on lab data, not real-world cases, so I wouldn't panic. Still, given Bulbine's possible liver and kidney concerns and the lack of human proof, I'd run it past your pharmacist or doctor before combining them.
