Erythromycin Gluceptate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the gist with this pairing. Erythromycin is cleared in part by a liver enzyme called CYP3A4. In lab and animal studies, Bulbine natalensis seems to rev that enzyme up (induce it). If that holds true in people, your body could break erythromycin down a little faster, which might leave less of the antibiotic working to clear your infection.

I want to be honest, though: this is purely theoretical right now, based on test-tube and rodent data, not real patients. So I wouldn't lose sleep over it. The bigger concern is that Bulbine itself has shown possible liver and kidney harm in animals, and erythromycin can be hard on the liver too. Please check with your pharmacist or doctor before combining them, and don't change how you take your antibiotic on your own.

Here's the situation with erythromycin (given here as the gluceptate IV form) and Bulbine natalensis. Erythromycin is carried around your body partly by a transporter pump called P-glycoprotein, which helps shuttle the drug out of cells. The concern on file is that Bulbine natalensis might affect that pump, which could in theory shift how much erythromycin stays in your system.

I want to be honest with you: this is a very weak, theoretical flag. It comes only from lab dish (in vitro) research, not from people, and the study didn't even confirm the pump actually changed. So I wouldn't lose sleep over it. Still, erythromycin can cause stomach upset, nausea, and rarely heart-rhythm changes, and Bulbine itself has raised liver and kidney concerns in animals. Please run this combo by your pharmacist or doctor first.
