Fedratinib Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the concern with pairing this supplement and your fedratinib (Inrebic). Lab studies suggest Bulbine natalensis may speed up an enzyme in your liver called CYP3A4, and fedratinib happens to be cleared partly by that same enzyme. Fedratinib is an active drug, not a prodrug, so if your body breaks it down faster, you could end up with lower drug levels. In theory that might mean it works less well against your myelofibrosis, which is not something you want to risk with a cancer medication.

That said, this is only test-tube evidence and has never been confirmed in people, so we don't really know how big the effect would be. Because fedratinib is a serious medication, please don't add this supplement without first checking with your oncologist or pharmacist.

What Is Fedratinib Used For?

Brand name: Inrebic

Inrebic (fedratinib) is approved to treat adult patients with intermediate-2 or high-risk myelofibrosis (MF) — a type of bone marrow cancer that disrupts normal blood cell production. This includes primary myelofibrosis as well as secondary myelofibrosis that developed after polycythemia vera or essential thrombocythemia.

It is used both in patients who have not been treated before and in those who have previously been treated with ruxolitinib, another myelofibrosis medication.
