Fluticasone, Salmeterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Both pieces of Advair, the steroid (fluticasone) and the long-acting bronchodilator (salmeterol), are broken down in your body by an enzyme called CYP3A4. In lab and animal tests, Bulbine natalensis seemed to speed up that enzyme. If the same thing happened in people, your body could clear these medicines a bit faster, which might make your inhaler work less well. For asthma or COPD, that could mean more wheezing, shortness of breath, or rescue inhaler use.

Here's the honest part: this is purely theoretical, based only on test-tube and animal data, with nothing shown in humans. Inhaled doses are also fairly low. Still, given the liver concerns with this supplement, please check with your pharmacist or doctor before combining them, and never change your Advair on your own.
