Ibrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the situation with ibrutinib (Imbruvica) and Bulbine natalensis. Your body clears ibrutinib using a liver enzyme called CYP3A4. Lab studies suggest this supplement may rev up that enzyme, which could speed up how fast ibrutinib is broken down. The worry isn't more side effects here, it's the opposite: lower ibrutinib levels could make it less effective against the cancer it's treating, and that matters a lot for a drug doing such important work.

I want to be honest about the strength of this. It comes only from test-tube research, not from people, so we can't say for sure it happens in real life. Still, given how serious ibrutinib's job is, I'd play it safe. Please run this by your oncologist or pharmacist before adding Bulbine natalensis.

Theoretically, combining Bulbine natalensis leaf with drugs that have anticoagulant or antiplatelet activity might increase the risk of bruising and bleeding.
In vitro research shows that Bulbine natalensis ground leaf extract inhibits platelet aggregation.

Here's the thing about ibrutinib (Imbruvica): even on its own, it can thin your blood a bit and raise the chance of bruising and bleeding. That's a known side effect of this cancer medicine, so doctors already keep an eye on it.

Now add Bulbine natalensis. In lab dish studies (not people), its leaf extract seemed to make blood platelets clump together less well. Stacked on top of ibrutinib, the worry is a little extra bleeding risk, like easy bruising, nosebleeds, or bleeding gums. To be fair, this is a theoretical concern based on test-tube data, not proven in patients. Still, with a drug like ibrutinib I'd play it safe. Please check with your oncologist or pharmacist before adding this supplement.

What Is Ibrutinib Used For?

Brand name: Imbruvica

Imbruvica (ibrutinib) is used in adults to treat chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL), including cases with a specific genetic change called 17p deletion. It is also used in adults to treat Waldenström's macroglobulinemia (WM), a rare type of slow-growing blood cancer.

Imbruvica is also approved for adults and children aged 1 year and older with chronic graft-versus-host disease (cGVHD) — a serious immune complication that can occur after a stem cell or bone marrow transplant — when at least one prior treatment has not worked.
