Letrozole, Ribociclib Succinate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the gist for your Kisqali Femara Co-Pack (which combines ribociclib and letrozole). Both of these cancer medicines are cleared by a liver enzyme called CYP3A4. In lab dishes, Bulbine natalensis appeared to speed up that enzyme. If that held true in people, your body might break these drugs down faster, leaving less in your system. Neither one is a prodrug, so faster clearance would mean weaker treatment, not stronger, and that's a serious concern with breast cancer therapy.

I want to be honest: this is only a theory from test-tube work, never confirmed in humans. But since this combo is treating cancer and Bulbine may also stress the liver, please don't add it without first clearing it with your oncologist or pharmacist.
