Mometasone, formoterol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP2C9 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP2C9 substrates; however, this has not been studied in humans.

Here's the honest picture with Zenhale (mometasone and formoterol). This is an inhaler you breathe in to keep asthma under control, so most of the medicine works right in your lungs rather than circulating through your liver. The concern on file is that Bulbine natalensis might speed up a liver enzyme called CYP2C9, which could in theory clear certain drugs faster and weaken them. If that happened with your inhaler, you could notice less control of your asthma, meaning more wheezing, coughing, or shortness of breath.

That said, this is only a lab and animal finding, not something proven in people, and inhalers act mostly at the lungs. I wouldn't lose sleep over it, but please run it by your pharmacist or doctor before adding Bulbine natalensis.

Your Zenhale inhaler combines two medicines: mometasone, an inhaled steroid that calms airway swelling, and formoterol, which opens up the airways. Your liver clears both using an enzyme called CYP3A4. The concern here is that Bulbine natalensis may speed up that enzyme, which could break these drugs down faster and leave less active medicine in your body.

If that happened, your asthma or COPD control might slip, meaning more wheezing, coughing, or shortness of breath. But I want to be honest: this is purely theoretical, based only on lab and animal studies, never tested in people. It's also unclear how much inhaled medicine this would even affect. Still, given the liver concerns with this supplement, please check with your doctor or pharmacist before combining them.
