Naloxegol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bulbine Natalensis interacts with.

Theoretically, Bulbine natalensis stem might reduce the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Bulbine natalensis stem extract induces CYP3A4 mRNA expression and enzyme activity. Theoretically, this may increase the metabolism of CYP3A4 substrates, but this has not been studied in humans.

Here's the gist for Movantik (naloxegol). Your liver uses an enzyme called CYP3A4 to break this medicine down. In lab and animal studies, Bulbine natalensis seemed to speed up that enzyme. If that also happened in people, your body might clear naloxegol faster, leaving less of it to do its job.

Naloxegol isn't a prodrug, so faster breakdown likely means a weaker effect, meaning your opioid-related constipation could come back. To be fair, this is only a theory from test-tube work, not proven in humans, so don't panic. Given that Bulbine also raises liver and kidney concerns, I'd run it by your pharmacist or doctor before combining the two.

What Is Naloxegol Used For?

Brand name: Movantik

Movantik (naloxegol) is used to treat opioid-induced constipation (OIC) — constipation caused by opioid pain medications — in adults who have chronic non-cancer pain. It is also approved for adults with chronic pain related to prior cancer or its treatment, as long as they don't need frequent (roughly weekly) increases in their opioid dose.

It is not indicated for constipation from other causes, and it is not approved for use in children.
