Alteplase, tPA falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Bupleurum interacts with.

Theoretically, bupleurum might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
In vitro research suggests that saikosaponins, constituents of bupleurum, can inhibit platelet aggregation.

Here's the thing to know about alteplase (tPA): it's a powerful clot-busting medicine given in the hospital during emergencies like a stroke or heart attack. Because it actively breaks down clots, bleeding is already its biggest risk. The concern with bupleurum is that lab studies suggest its saikosaponins may slow down platelets, the cells that help your blood clot. Stack that on top of tPA and, in theory, you could tip the scales toward more bleeding.

I want to be honest though: this is a theoretical worry based on test-tube research, not real patient cases. Since tPA is given in a controlled hospital setting, this is something your care team would weigh, not something to manage on your own. Always mention any herbs you take to your doctor or pharmacist.
