Kombiglyze pairs saxagliptin with extended-release metformin, and both are working to bring your blood sugar down. Here's the wrinkle with bupleurum: in animal studies, its saikosaponin compounds appeared to raise blood glucose. If that holds true in people, the herb could push in the opposite direction of your medication and make it work less well.

To be clear, this is a theoretical concern based mostly on lab and animal data, not strong human studies. Still, it's worth watching. If you take bupleurum alongside Kombiglyze, keep an eye on your home glucose readings for higher-than-usual numbers or symptoms like extra thirst, frequent urination, or fatigue. Before adding this herb, please run it by your pharmacist or doctor so they can help you monitor.
