Calcium D-glucarate and Acetaminophen, Hydrocodone

Here is the interaction we found between these two, with the evidence behind it.

Moderate

GLUCURONIDATED DRUGS

Why this applies to Acetaminophen, Hydrocodone

Acetaminophen, Hydrocodone falls under GLUCURONIDATED DRUGS — the group Calcium D-glucarate interacts with.

Theoretically, calcium D-glucarate might increase the clearance of drugs that undergo glucuronidation.
The calcium D-glucarate metabolite, D-glucaro-1,4-lactone, inhibits the enzyme beta-glucuronidase, reducing deconjugation of glucuronides in the intestine and thereby reducing reabsorption of the drugs.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the idea behind this pairing. Both the acetaminophen and the hydrocodone in your Vicodin or Norco get partly processed in the liver by being tagged with a molecule (glucuronidation) so your body can flush them out. Calcium D-glucarate may block an enzyme in the gut that normally recycles some of that drug back into your blood. By stopping the recycling, it could, in theory, help your body clear these medicines a little faster.

If that happened, you might notice your pain relief from the hydrocodone wearing off sooner or feeling weaker. Keep in mind this is mostly theoretical and based on limited research, not strong human studies, so for most people the effect is likely small. Still, before you combine them, it's worth a quick chat with your pharmacist or doctor.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
