Calcium D-glucarate and Deferasirox

Here is the interaction we found between these two, with the evidence behind it.

Moderate

GLUCURONIDATED DRUGS

Why this applies to Deferasirox

Deferasirox falls under GLUCURONIDATED DRUGS — the group Calcium D-glucarate interacts with.

Theoretically, calcium D-glucarate might increase the clearance of drugs that undergo glucuronidation.
The calcium D-glucarate metabolite, D-glucaro-1,4-lactone, inhibits the enzyme beta-glucuronidase, reducing deconjugation of glucuronides in the intestine and thereby reducing reabsorption of the drugs.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Deferasirox (Exjade, Jadenu). Your body clears this iron-removing medicine partly by attaching a sugar-like tag to it (a process called glucuronidation) so it can leave in your stool and urine. Calcium D-glucarate may block an enzyme in your gut that normally peels that tag back off, which means less of the drug gets reabsorbed and a bit more gets flushed out. In plain terms, it could slightly lower deferasirox levels.

If levels dip, the medicine may not pull out as much excess iron over time. Just so you know, this is mostly a theoretical concern based on lab reasoning, not strong human studies, so don't panic. Still, since controlling iron matters, check with your pharmacist or doctor before pairing them.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About Calcium D-glucarate

What is Calcium D-glucarate?

Calcium D-glucarate
  • Detoxification support
  • Hormone balance
  • Estrogen metabolism
  • Liver support
  • General wellness

Calcium D-glucarate is a supplement form of a natural compound found in fruits and vegetables that is promoted for 'detoxification' and helping the body clear excess hormones. Human evidence for these benefits is very limited, and most research comes from laboratory or animal studies. Talk to your pharmacist or doctor before using it, especially if you have a hormone-related condition.

About Deferasirox

What Is Deferasirox Used For?

Brand names: Exjade, Jadenu

Deferasirox treats chronic iron overload, meaning too much iron has built up in the body over time. The label supports two uses.

The first is iron overload from blood transfusions, in patients 2 years and older. The second is iron overload in non-transfusion-dependent thalassemia (NTDT) syndromes, in patients 10 years and older. These patients must also have a high liver iron concentration and a serum ferritin above 300 mcg/L. It has not been shown to be safe or effective when combined with other iron chelation therapy.
