Macitentan, Tadalafil falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Cannabinol (cbn) interacts with.

Theoretically, cannabinol might increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that cannabinol potently inhibits CYP2C9 enzymes. So far, this interaction has not been reported in humans.

Here's the gist with Opsynvi, which pairs macitentan with tadalafil. In lab dishes, CBN can block a liver enzyme called CYP2C9 that helps clear the tadalafil part of this combo. If that enzyme slows down, tadalafil could build up a bit more than expected, which might mean stronger side effects like headache, flushing, stuffy nose, dizziness, or a bigger drop in blood pressure.

I want to be honest about the evidence though: this is only test-tube data, and it has not actually been reported in real people. So it's more of a theoretical caution than a proven problem. Since Opsynvi is a serious lung and heart medicine, please don't start CBN on your own. Run it by your pharmacist or doctor first.
