Voxelotor falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Cannabinol (cbn) interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of cannabinol.
In vitro research shows that CYP3A4 enzymes are involved in the 8-hydroxylation of cannabinol. So far, this interaction has not been reported in humans.

Here's the thing to understand about this pairing: voxelotor (Oxbryta) acts as a CYP3A4 inhibitor, meaning it can slow down one of the enzymes your liver uses to break things down. Lab studies suggest CBN is partly cleared by that same CYP3A4 enzyme. So in theory, voxelotor could let CBN build up a bit higher than expected, which might make CBN's effects (like drowsiness or grogginess) stronger.

I want to be honest about how thin this evidence is. This comes only from test-tube research, and it has never actually been reported in real people. Voxelotor itself isn't the one being affected here, so your sickle cell treatment shouldn't change. Still, since CBN products vary a lot in quality, please run it by your pharmacist or doctor first.

Oxbryta (voxelotor) is approved to treat sickle cell disease (SCD) in adults and in children as young as 4 years old. It works by targeting the underlying problem in sickle cell disease — the abnormal hemoglobin that causes red blood cells to take on a rigid, sickle shape.
