Chlorpheniramine, Phenylephrine, Phenylpropanolamine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabis interacts with.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination cold product (Rutuss) blends an antihistamine, chlorpheniramine, with two decongestants. Chlorpheniramine is partly broken down by the liver enzyme CYP3A4. The lab data we have suggest cannabis may slow that enzyme down a bit, which in theory could let chlorpheniramine build up slightly. If that happened, you might feel more of its usual effects, like drowsiness, dizziness, dry mouth, or a foggy head, especially since cannabis can already make you sleepy and impaired on its own.
Keep in mind this is only test-tube evidence, not proven in real people, so the real-world effect is likely small. Still, the decongestant pieces can raise heart rate and blood pressure, so please check with your pharmacist or doctor before mixing these.
