Why this applies to Pemigatinib

Pemigatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabis interacts with.

Theoretically, cannabis may increase the levels and adverse effects of CYP3A4 substrates.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the basic idea: your body uses a liver enzyme called CYP3A4 to clear pemigatinib (Pemazyre) out of your system. Lab studies suggest some cannabis compounds, especially CBD and CBG, can slow this enzyme down a bit. If that enzyme works more slowly, pemigatinib could build up to higher levels than expected, which may mean more of its side effects, like dry mouth, diarrhea, hair or nail changes, mouth sores, fatigue, joint pain, or eye problems.

I want to be honest with you, though: this concern comes only from test-tube research, not real patient data, so we don't truly know how much it matters in people. Since pemigatinib is a serious cancer drug, please don't change anything on your own. Just have a quick chat with your oncologist or pharmacist first.

About Pemigatinib

What Is Pemigatinib Used For?

Brand name: Pemazyre

Pemazyre (pemigatinib) is used to treat adults with previously treated, unresectable locally advanced or metastatic cholangiocarcinoma (bile duct cancer) whose tumors have a specific genetic change called an FGFR2 fusion or rearrangement, confirmed by an FDA-approved test. This approval is based on tumor response rates; continued approval may depend on results from further confirmatory studies.

Pemazyre is also approved to treat adults with relapsed or refractory myeloid/lymphoid neoplasms (MLNs) — certain blood and bone marrow cancers — that carry an FGFR1 rearrangement and have come back or stopped responding to prior treatment.
