Aspirin, Codeine Phosphate, Phenobarbital falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Clove interacts with.

Theoretically, concomitant use of clove may increase levels of drugs metabolized by CYP2D6.
In vitro research shows that eugenol, the principal constituent of clove, can inhibit CYP2D6 in a dose-dependent manner. This effect has not been reported in humans.

Here's the wrinkle with this combo product: the codeine portion is a prodrug. Your body has to switch it on using an enzyme called CYP2D6, which turns codeine into morphine, the part that actually relieves pain. Lab and animal studies suggest eugenol (the main compound in clove) can slow down CYP2D6. If that happened in people, less codeine would get converted, so you might get weaker pain relief rather than more.

I want to be honest about how thin the evidence is. This has only been seen in test tubes, not in real patients, and a sprinkle of clove in cooking is very unlikely to do anything. Concentrated clove oil or capsules are the bigger question mark. The aspirin and phenobarbital pieces aren't really affected here. If you use clove medicinally, just run it by your pharmacist or doctor first.

The piece of this combo pill that matters here is the aspirin. Aspirin already thins your blood a bit by keeping platelets from clumping together. Lab studies suggest that eugenol, the main oil in clove, may do something similar. So in theory, stacking concentrated clove on top of aspirin could nudge your bleeding risk up a little, meaning easier bruising, nosebleeds, or bleeding that takes longer to stop.

I want to be honest about how weak this is. It is only seen in test-tube research and has never actually been reported in people. The clove in your cooking is no concern at all. The caution is really for clove oil or concentrated supplements. If you use those regularly, just give your pharmacist or doctor a heads-up first.
