Coenzyme Q10 and Sodium Nitroprusside

Here is the interaction we found between these two, with the evidence behind it.

Minor

ANTIHYPERTENSIVE DRUGS

Why this applies to Sodium Nitroprusside

Sodium Nitroprusside falls under ANTIHYPERTENSIVE DRUGS — the group Coenzyme Q10 interacts with.

Theoretically, coenzyme Q10 might have additive effects with antihypertensive drugs.
Some clinical research shows that coenzyme Q10 can significantly lower blood pressure, although other studies have shown conflicting results.
Severity Minor Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the practical side of this one. Sodium nitroprusside isn't something you'd ever take at home. It's a powerful IV drip used in a hospital or ICU to bring down dangerously high blood pressure, and the staff adjust it minute by minute while watching your numbers closely. CoQ10, on its own, may nudge blood pressure down a little in some people, though the studies are mixed and the effect is modest.

So in theory, adding CoQ10 could slightly add to the blood pressure lowering from nitroprusside, possibly contributing to dizziness or low pressure. In reality, your care team is already titrating the drip in real time, so this overlap is very unlikely to cause a problem. Still, let them know about any supplements you take so nothing surprises them.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Sodium Nitroprusside page

“Sodium nitroprusside (Nipride RTU) is given by IV in hospitals to bring dangerously high blood pressure down within minutes, so anything that nudges pressure in either direction earns real vigilance here. All but two of the 121 supplements and herbs on this page interact through blood pressure itself, and the three major-rated entries…”
