Here's the thing to know about this combo. Your medicine includes hydrochlorothiazide, a water pill that already pulls potassium out of your body through your kidneys. Colocynth works as a harsh laxative, and lots of watery stools can drain potassium too. Stack those two together and your potassium could drop too low. Low potassium (hypokalemia) can show up as muscle cramps, weakness, fatigue, or an irregular heartbeat.

This warning is theoretical, based on how these things work rather than strong human studies. But honestly, colocynth is also genuinely toxic and most of us would steer you away from it entirely. Please check with your pharmacist or doctor before using any product that contains it.
