Acetaminophen, Codeine, Doxylamine falls under CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP2E1 substrates.
Chlorzoxazone is metabolized by CYP2E1 enzymes. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of chlorzoxazone (Paraflex 250) 250 mg increases blood levels of chlorzoxazone by 34% and decreases chlorzoxazone clearance by 40%. It is speculated that diosmin reduces the metabolism of chlorzoxazone by inhibiting CYP2E1.

Here's the practical angle for Mersyndol (which combines acetaminophen, codeine, and doxylamine). Diosmin may gently slow down a liver enzyme called CYP2E1. For most of these ingredients, that's not a big deal. The part worth a mention is the acetaminophen (Tylenol) piece. CYP2E1 is the same enzyme that turns a small slice of acetaminophen into a substance that can stress the liver. In theory, slowing that pathway might actually reduce that toxic byproduct a little, so this is not a reason to worry about your usual dose.

This is only a theoretical concern based on how the enzyme works, not from real-world reports of harm, so I wouldn't lose sleep over it. The bigger thing with this product is simply not exceeding the acetaminophen limit and watching for drowsiness from the codeine and doxylamine. Check with your pharmacist before combining.
