Ibuprofen lysine falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might inhibit the metabolism of CYP2C9 substrates.
Diclofenac is metabolized by CYP2C9 enzymes. Clinical and laboratory research shows that diosmin inhibits the metabolism of diclofenac. A pharmacokinetic study in humans shows that taking diosmin (Venex 500) 500 mg daily for 9 days prior to oral administration of diclofenac 100 mg increases blood levels of diclofenac and decreases diclofenac clearance.

Here's the gist: ibuprofen (the active part of ibuprofen lysine) is cleared from your body partly by a liver enzyme called CYP2C9. Diosmin is thought to slow that same enzyme down. If it does, your body would break ibuprofen down a little more slowly, so more of it could linger in your blood. That isn't ibuprofen being activated; it's already active, so the practical concern is simply a bit more drug hanging around than usual.

What might that look like? The familiar ibuprofen side effects could show up more easily, things like stomach upset, heartburn, or, with regular use, irritation or bleeding in the gut and effects on the kidneys. Keep in mind this is a theoretical, lab-and-pharmacology based concern, not something proven to cause harm in people taking the two together. Still, it's worth a quick chat with your pharmacist or doctor before combining them.

Theoretically, diosmin may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
A case of spontaneous intraventricular hemorrhage has been reported for a 77-year-old female after 6 weeks of warfarin therapy, despite an international normalized ratio (INR) of only 1.8. The patient had also been taking aspirin and diosmin for several years. Experts speculate that chronic intake of diosmin predisposed the patient to spontaneous intraventricular hemorrhage by inducing chronic microcirculatory hypertension and inhibiting platelet aggregation. The presence of aspirin was also thought to play a role in this event.

Here's the gist: ibuprofen lysine is just a fast-dissolving form of ibuprofen, an NSAID. On its own, ibuprofen can make your blood a little less likely to clot and can irritate the stomach lining, so it already carries some bleeding risk. Diosmin, the citrus flavonoid, may have a mild blood-thinning effect of its own, so the worry is that the two together could add up and slightly raise your chance of bleeding or bruising.

I want to be honest about how thin this evidence is. It's mostly theoretical, built on one older case report in a person also taking warfarin and aspirin, not ibuprofen alone. For most people this is a low-level concern. Still, watch for unusual bruising, nosebleeds, dark stools, or stomach pain, and check with your pharmacist or doctor before pairing them, especially if you take other blood thinners.

About Ibuprofen

What Is Ibuprofen Used For?

Brand name: NeoProfen

Oral ibuprofen products (tablets, coated tablets, and liquid-filled capsules) are used to temporarily relieve minor aches and pains from headache, backache, toothache, muscular aches, menstrual cramps, minor arthritis pain, and the common cold, and to temporarily reduce fever. The oral prescription tablet (Ibu) is also indicated for the signs and symptoms of rheumatoid arthritis and osteoarthritis, mild to moderate pain, and primary dysmenorrhea (painful periods).

Caldolor, an intravenous (IV) form, is approved for adults and children 3 months and older to manage mild to moderate pain, to manage moderate to severe pain alongside opioid pain medicines, and to reduce fever. A separate intravenous solution, Ibuprofen Lysine, is used specifically in premature infants to close a patent ductus arteriosus (PDA) — an abnormal blood vessel connection near the heart — when standard medical management has not worked.
