Why this applies to Cimetidine

Cimetidine falls under P-GLYCOPROTEIN SUBSTRATES — the group Diosmin interacts with.

Theoretically, diosmin might increase levels of drugs that are substrates of P-glycoprotein (P-gp).
Preliminary laboratory research suggests that diosmin inhibits P-gp. Additionally, pharmacokinetic research in healthy male subjects shows that taking diosmin (Venex) 500 mg daily for 10 days prior to oral administration of fexofenadine 120 mg increases blood levels of fexofenadine, a P-gp substrate, by approximately 49% and decreases the apparent oral clearance of fexofenadine by 41%. The time taken to reach maximum plasma concentration, the half-life, and the apparent renal clearance of fexofenadine are not affected. For this reason, it is speculated that diosmin inhibits P-gp in the intestine, but not in the kidney or liver.

Here's the gist: cimetidine is moved around your gut by a little cellular pump called P-glycoprotein, and diosmin appears to slow that pump down. In theory, that could let a bit more cimetidine get absorbed and raise its levels in your blood. Cimetidine is not a prodrug, so more in your system means a slightly stronger effect, and possibly more of its usual side effects like headache, dizziness, or loose stools.

The honest part: this is based mainly on lab and early human studies (the clearest data came from a different drug, fexofenadine), so it's more theoretical than proven for cimetidine. For most people taking standard heartburn doses, it's unlikely to cause real trouble. Still, mention to your pharmacist that you're taking both so they can keep an eye out.

Diosmin is a citrus-derived flavonoid most often used for vein-related problems such as chronic venous insufficiency and hemorrhoids, and it is usually combined with a related flavonoid called hesperidin. Some studies suggest it may help relieve leg swelling and hemorrhoid symptoms, but quality varies. What Is Cimetidine Used For?

Brand names: Tagamet HB, Tagamet, Cimetidine Injection

Cimetidine tablets sold over the counter — including brands like Acid Controller, Acid Reducer, Basic Care Acid Reducer, Equate Cimetidine, and cimetidine 200 — are used to relieve heartburn associated with acid indigestion and sour stomach, and to prevent heartburn brought on by eating or drinking certain foods and beverages.

Prescription-strength cimetidine tablets and Cimetidine Hydrochloride Oral Solution are used for more serious conditions: short-term treatment of active duodenal ulcers (ulcers in the upper small intestine) and benign gastric ulcers (stomach ulcers), maintenance therapy to keep duodenal ulcers from coming back, healing of erosive GERD (acid reflux that has damaged the lining of the esophagus), and treatment of conditions that cause the stomach to make too much acid — such as Zollinger-Ellison Syndrome, systemic mastocytosis, and multiple endocrine adenomas.
