Hydrochlorothiazide, Losartan Potassium falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.

Here's the situation with Hyzaar (a combo of losartan and hydrochlorothiazide) and eleuthero. The theory is that eleuthero might slow down a liver enzyme called CYP3A4, which could let certain medicines build up a bit. Losartan does use this enzyme pathway, and interestingly losartan is a prodrug that gets converted into its more active form partly through liver enzymes, so the picture isn't simple.

But here's the reassuring part: when researchers actually tested eleuthero in healthy people, it did not meaningfully block CYP3A4. So this is mostly a lab and animal concern, and a real-world problem is unlikely. Still, if you ever feel unusually dizzy, lightheaded, or notice your blood pressure running low, check in with your pharmacist or doctor.
