Methadone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP2D6.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP2D6. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP2D6 drug metabolism.

Here's the short version: on paper, eleuthero might slow down an enzyme called CYP2D6, which is one of the pathways your body uses to clear methadone. If that happened, methadone could build up a little, and you might notice more of its usual effects, things like extra drowsiness, slowed breathing, constipation, or feeling lightheaded.

But the real-world evidence is reassuring. When researchers gave healthy people eleuthero twice a day for two weeks, it did not meaningfully slow CYP2D6. So this is mostly theoretical, the odds of a problem are low, and any effect would likely be minor. Still, methadone is a powerful medicine with a narrow safety margin, so before you add eleuthero, it's worth a quick check with your pharmacist or doctor.

Here's the short version on combining eleuthero with methadone. Your liver uses an enzyme called CYP3A4 to help clear methadone. In test tubes and animal studies, eleuthero looked like it might slow that enzyme down, which in theory could let methadone build up and lead to stronger opioid effects like extra drowsiness, slowed breathing, or constipation. Methadone is one of those drugs where even small changes deserve respect.

The good news: when researchers actually gave healthy people eleuthero (485 mg twice a day for two weeks), it did not meaningfully change CYP3A4 activity. So this is mostly theoretical and unlikely to cause real trouble. Still, methadone is serious medicine, so check with your pharmacist or prescriber before adding eleuthero.

About Methadone

What Is Methadone Used For?

Brand name: Dolophine

Methadone treats severe and persistent pain that requires daily, around-the-clock, long-term opioid treatment and cannot be managed adequately with other options. It is available in several forms for this use, including oral solution, oral tablets, oral concentrate, and injectable forms (intravenous, intramuscular, and subcutaneous). Injectable methadone is not approved for outpatient pain treatment — it is reserved for hospitalized patients unable to take oral medication.

Methadone is also used for the detoxification and maintenance treatment of opioid addiction (dependence on heroin or other morphine-like drugs). Oral forms — including tablets, dispersible tablets (such as Methadose Dispersible and DISKETS), and oral concentrates (such as Methadone Hydrochloride Intensol™, Methadose, and others) — are approved for this use. By law, methadone for opioid addiction treatment must be dispensed only by certified opioid treatment programs. Injectable methadone is not approved for outpatient addiction treatment.
