Why this applies to Methadone

Methadone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist for methadone. Lab studies suggest plumbagin, a compound in Ceylon Leadwort, can block an enzyme called CYP2D6, which is one of the pathways that helps your body break methadone down. In theory, if that pathway slows, more methadone could linger in your system, nudging up its effects and side effects (think extra drowsiness, dizziness, slowed breathing, or constipation).

That said, this is only test-tube evidence and has not been seen in real people. Methadone also leans heavily on a different enzyme (CYP3A4), so CYP2D6 plays a smaller role. Still, methadone is a powerful opioid where small shifts matter, so please loop in your pharmacist or doctor before combining the two.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to help clear methadone. A compound in Ceylon Leadwort called plumbagin has been shown to slow that enzyme down, but only in lab dish studies so far, never in actual people. In theory, slowing CYP3A4 could let methadone build up more than usual.

Methadone is a drug where I get extra cautious, because too much can cause heavy sedation, slowed or shallow breathing, and dangerous heart rhythm changes (QT prolongation). The evidence here is weak and theoretical, so I'm not trying to scare you, but methadone leaves little room for error. Please loop in your pharmacist or methadone clinic before adding this supplement so they can keep you safe.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Methadone

What Is Methadone Used For?

Brand name: Dolophine

Methadone treats severe and persistent pain that requires daily, around-the-clock, long-term opioid treatment and cannot be managed adequately with other options. It is available in several forms for this use, including oral solution, oral tablets, oral concentrate, and injectable forms (intravenous, intramuscular, and subcutaneous). Injectable methadone is not approved for outpatient pain treatment — it is reserved for hospitalized patients unable to take oral medication.

Methadone is also used for the detoxification and maintenance treatment of opioid addiction (dependence on heroin or other morphine-like drugs). Oral forms — including tablets, dispersible tablets (such as Methadose Dispersible and DISKETS), and oral concentrates (such as Methadone Hydrochloride Intensol™, Methadose, and others) — are approved for this use. By law, methadone for opioid addiction treatment must be dispensed only by certified opioid treatment programs. Injectable methadone is not approved for outpatient addiction treatment.
