Fluvoxamine falls under SEROTONERGIC DRUGS — the group Ergot interacts with.

Certain ergot alkaloids, such as dihydroergotamine, act as serotonin agonists. Theoretically, combining serotonergic drugs with ergot might increase the risk of serotonergic side effects including serotonin syndrome and cerebral vasoconstrictive disorders. Monitor patients for signs of serotonin syndrome and other serotonergic side effects if using ergot with serotonergic drugs.
Serotonergic drugs include the selective serotonin reuptake inhibitors (SSRIs) such as fluoxetine (Prozac), paroxetine (Paxil), and sertraline (Zoloft); tricyclic and atypical antidepressants such as amitriptyline (Elavil), clomipramine (Anafranil), and imipramine (Tofranil); triptans such as sumatriptan (Imitrex), zolmitriptan (Zomig), and rizatriptan (Maxalt); opioids such as methadone (Dolophine) and tramadol (Ultram); and many other medications.

Here's the simple version: fluvoxamine (Luvox) raises serotonin levels in your brain, which is exactly how it eases OCD and social anxiety. Ergot alkaloids also nudge serotonin-type receptors. Stack the two together and you could push serotonin signaling too high. That can lead to serotonin syndrome, with symptoms like agitation, a racing heart, sweating, shivering, muscle twitching, or confusion. Ergot also strongly tightens blood vessels, which is a separate concern on its own.

I'll be honest, this warning is mostly theoretical and based on scattered case reports, not strong proof. But here's the bigger point: raw ergot is genuinely toxic and was never meant to be a supplement. Please don't take it. If a doctor ever prescribes an ergot medicine while you're on fluvoxamine, let your pharmacist know first.

Ergotamine, a constituent of ergot, is a substrate of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that inhibit CYP3A4 might increase the risk of ergot toxicity. Some of these drugs include amiodarone (Cordarone), clarithromycin (Biaxin), diltiazem (Cardizem), erythromycin (E-mycin, Erythrocin), indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase), and many others.

Here's the thing with fluvoxamine (Luvox): it's a strong blocker of a liver enzyme called CYP3A4. Ergot's active chemical, ergotamine, is broken down by that very enzyme. So if you slow down that cleanup crew with fluvoxamine, ergot can pile up in your body and hit harder than it should.

With too much ergot on board, the worry is ergot toxicity: blood vessels clamping down too tightly, causing things like cold, numb, or tingling fingers and toes, leg cramps, or a pounding headache. Honestly, raw ergot isn't safe to take on its own anyway. This particular warning rests on weak, theoretical evidence, so don't panic, but please run any ergot product by your pharmacist or doctor first.

Ergot is a toxic fungus that grows on rye and contains powerful chemicals (ergot alkaloids) that strongly narrow blood vessels and squeeze the uterus. Because raw ergot is poisonous and the dose is impossible to control safely, it should never be used as a home remedy or supplement. Some purified ergot chemicals are used as prescription drugs under strict medical supervision, but the crude herb is not safe to take.

Both fluvoxamine maleate tablets and Fluvoxamine Maleate extended-release capsules are approved to treat obsessive-compulsive disorder (OCD) — a condition marked by unwanted, recurring thoughts (obsessions) and/or repetitive behaviors (compulsions) that cause significant distress or interfere with daily life.

Fluvoxamine maleate tablets are approved for adults and for children and adolescents ages 8 to 17. Fluvoxamine Maleate extended-release capsules are approved for adults with OCD; their use in pediatric patients naive to fluvoxamine requires special prescriber consideration regarding the lowest available dose.
