Amitriptyline, Perphenazine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Evodia interacts with.

Evodia might reduce the levels and clinical effects of CYP1A2 substrates through induction of CYP1A2.
Evodia extract and the evodia constituent rutaecarpine induce hepatic CYP1A2 enzyme activity. Evodia decreases levels of theophylline and caffeine, CYP1A2 substrates, by about 70% in animal models.

This combination pill pairs amitriptyline (an antidepressant) with perphenazine (an antipsychotic). The liver enzyme CYP1A2 helps clear amitriptyline from your body. Evodia appears to speed that enzyme up, so in theory it could push amitriptyline out faster and leave you with lower levels. That might mean less relief from depression, low mood, or nerve pain, basically the medicine not working as well as it should.

Honestly, the evidence here is thin. It comes from lab and animal studies, not real patients, so we can't be sure how much it matters for you. Still, since these are serious mood medicines, it's worth a quick chat with your pharmacist or doctor before adding Evodia rather than guessing.

Evodia has demonstrated dose-dependent activity as a proarrhythmic agent in animal and in vitro studies. Evodia infusion in animals extends the action duration potential and induces prolongation of the QT interval and Torsade de pointes.

Here's the connection worth knowing about. Triavil combines amitriptyline (an older antidepressant) with perphenazine (an antipsychotic), and both of these can stretch out a part of your heartbeat called the QT interval. Evodia has shown, mostly in test tube and animal studies, that it may do the same thing. Stack them together and the concern is that the effect could add up, raising the risk of an irregular heart rhythm. In rare cases that can mean palpitations, dizziness, fainting, or a dangerous arrhythmia.

I want to be honest that this is a theoretical worry based on lab and animal work, not strong human data. Still, because Triavil already carries its own heart-rhythm caution, I'd play it safe and check with your pharmacist or doctor before adding Evodia.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

This combination pill pairs amitriptyline (an antidepressant) with perphenazine (an antipsychotic), and both rely partly on liver enzymes (including CYP3A4) to be cleared from your body. Lab studies suggest evodia may block CYP3A4. If that happened in people, these medicines could build up a bit and their effects could become stronger.

In real terms, that might mean more drowsiness, dizziness, dry mouth, constipation, blurred vision, or a faster heartbeat. Here's the honest part though: this concern comes only from test-tube research and has not been shown in humans, so it's theoretical. Still, since both drugs already carry these side effects, it's worth a quick chat with your pharmacist or doctor before adding evodia.
