Evodia and Aspirin, Pentazocine

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Evodia interacts with whole categories of medication. Aspirin, Pentazocine falls into 2 of those categories, so it can interact with Evodia in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 2

Why this applies to Aspirin, Pentazocine

Aspirin, Pentazocine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Evodia interacts with.

Evodia might reduce the levels and clinical effects of CYP1A2 substrates through induction of CYP1A2.
Evodia extract and the evodia constituent rutaecarpine induce hepatic CYP1A2 enzyme activity. Evodia decreases levels of theophylline and caffeine, CYP1A2 substrates, by about 70% in animal models.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

The product you're asking about, Talwin Compound, combines aspirin with the pain reliever pentazocine. The concern here is that evodia may speed up a liver enzyme called CYP1A2, and pentazocine is partly handled by that pathway. If evodia revs up that enzyme, your body could clear the pentazocine a little faster, which might mean less pain relief than you expect.

I want to be honest about the strength of this, though. The evidence comes only from test tube and animal studies, not from people, so it's more of a theoretical heads-up than a proven problem. If you take Talwin Compound regularly and are thinking about adding evodia, just run it by your pharmacist or doctor first.

ANTICOAGULANT/ANTIPLATELET DRUGS

2 of 2

Aspirin, Pentazocine also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Theoretically, taking evodia with antiplatelet or anticoagulant drugs might increase the risk of bruising and bleeding.
In vitro and animal studies show that rutaecarpine, a constituent of evodia, inhibits platelet aggregation.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

The combo product you're asking about (Talwin Compound) pairs aspirin with the pain reliever pentazocine. The piece that matters here is the aspirin, since it already thins your blood by keeping platelets from clumping together. Lab and animal research suggests one of evodia's compounds, rutaecarpine, may also slow platelet activity. Stacked on top of aspirin, the worry is that bleeding could become a little easier to trigger.

In real life that might look like easier bruising, nosebleeds, bleeding gums, or a cut that takes longer to stop. To be honest, this is theoretical and based only on test-tube and animal studies, so it's far from proven. Still, I'd run any evodia supplement past your pharmacist or doctor first, and don't change how you take your aspirin product on your own.

From our pharmacist’s overview on the Aspirin, Pentazocine page

“Talwin Compound pairs pentazocine, an opioid painkiller, with aspirin, and I read its supplement list with more concern than most. The great majority of entries do one of two things: raise the bleeding risk that aspirin already carries, or deepen the sedation pentazocine causes.”
