Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the thing worth knowing if you take Teczem (diltiazem plus enalapril) and are curious about evodia. The diltiazem half of your medicine is a known blocker of a liver enzyme called CYP3A4. Evodia's active compounds (like evodiamine) are partly broken down by that same enzyme. So when you slow that enzyme down, evodia can build up in your system more than expected.

In plain terms, the worry here is the supplement getting stronger, not your blood pressure medicine. The catch is that this comes only from lab and animal research, so it's theoretical and not proven in people. Evodia itself isn't well studied, so I'd skip it or check with your pharmacist or doctor before combining the two.

Here's the thing to know about this pairing: the concern runs toward the Evodia, not your medication. Your combo pill (the diltiazem part especially) can gently slow down a liver enzyme called CYP1A2, and that same enzyme is what your body uses to break down rutaecarpine, one of Evodia's active compounds. So in theory, taking them together could let more Evodia build up in your system, possibly making its effects and any side effects a bit stronger.

I'll be honest with you, this is based only on lab and animal research, so it's more of a 'good to know' than a red alarm. Your diltiazem and enalapril should keep working as expected. Still, before adding Evodia, run it by your pharmacist or doctor first.

In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

This combination pill pairs diltiazem with enalapril, and it's the diltiazem part that matters here. Your liver uses an enzyme called CYP3A4 to clear diltiazem out of your system. In lab studies (test tubes, not real people), evodia appears to slow that same enzyme down. If that held true in the body, diltiazem could build up a bit higher than expected.

What might that feel like? Possibly more of diltiazem's usual effects, like low blood pressure, dizziness, a slower heart rate, or swollen ankles. I want to be honest though: this is theoretical and hasn't actually been seen in humans, so I wouldn't panic. Still, since blood pressure and heart rate are involved, it's worth a quick chat with your pharmacist or doctor before adding evodia.
