Evodia extract and the evodia constituent rutaecarpine induce hepatic CYP1A2 enzyme activity. Evodia decreases levels of theophylline and caffeine, CYP1A2 substrates, by about 70% in animal models.

This combination product contains theophylline, which your liver clears using an enzyme called CYP1A2. Evodia appears to speed that enzyme up. In animal studies, it dropped theophylline levels by about 70 percent. If that happened in people, your body could clear theophylline faster, leaving less in your blood than your doctor intended.

For a medicine like theophylline, that's worth caring about. It has a narrow safe range, and if levels fall too low, your breathing symptoms (wheezing, shortness of breath) might not stay controlled. Keep in mind this is based on lab and animal research only, not human studies. Still, please don't add Evodia without checking first with your pharmacist or doctor.

Theoretically, evodia might decrease the levels and clinical effects of theophylline.
The evodia constituent rutaecarpine decreases theophylline levels and half-life by about 70% in animal models. This constituent appears to induce hepatic cytochrome P450 1A2 (CYP1A2) enzyme activity, of which theophylline is a substrate. Rutaecarpine is the primary active constituent of evodia; however, it is not known if the whole crude extract of evodia also causes this interaction.

The part of your combo medicine I want to focus on is the theophylline, which helps open up your airways so you can breathe easier. Your liver clears theophylline using an enzyme called CYP1A2. A compound in evodia called rutaecarpine seems to speed that enzyme up, so your body could break theophylline down faster and leave less of it working. In other words, evodia may weaken your theophylline, and you might notice more wheezing, cough, or shortness of breath if it drops too low.

Here's the honest part: this finding comes from animal studies, not people, and we don't know if whole evodia extract does the same thing. Theophylline is also a tricky drug where levels matter a lot, so please check with your pharmacist or doctor before combining them.
