Imipramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Evodia interacts with.

Evodia might reduce the levels and clinical effects of CYP1A2 substrates through induction of CYP1A2.
Evodia extract and the evodia constituent rutaecarpine induce hepatic CYP1A2 enzyme activity. Evodia decreases levels of theophylline and caffeine, CYP1A2 substrates, by about 70% in animal models.

Here's the gist for imipramine (Tofranil): your liver uses an enzyme called CYP1A2 to help break this medicine down. Evodia appears to speed up that enzyme, at least in lab and animal studies. If it does the same in people, your body could clear imipramine faster and leave less of it in your system.

Imipramine isn't a prodrug, so lower levels likely mean weaker effects. That could look like your depression creeping back, or in children using it for bedwetting, less benefit there too. The good news is this is graded weak evidence (mostly test tube and animal work), so it's a theoretical concern, not a proven problem. Still, before pairing the two, check with your pharmacist or doctor so they can watch how you're doing.

Theoretically, evodia might have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias.
Evodia has demonstrated dose-dependent activity as a proarrhythmic agent in animal and in vitro studies. Evodia infusion in animals extends the action duration potential and induces prolongation of the QT interval and Torsade de pointes.

Here's the concern with pairing Evodia and imipramine. Like other tricyclic antidepressants, imipramine can stretch out the heart's electrical cycle (what doctors call the QT interval). In lab and animal studies, Evodia seemed to do the same thing. Stack two things that nudge the heart in that direction and, in theory, you raise the odds of an abnormal heart rhythm, including a dangerous one called Torsade de pointes.

I want to be honest about the evidence here. This is based on test tube and animal research, not real-world human cases, so it's a theoretical caution rather than a proven problem. Still, since imipramine already affects the heart's rhythm on its own, I'd play it safe. Please check with your pharmacist or doctor before combining the two, especially if you have any heart history.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the gist for imipramine (Tofranil). Your liver uses an enzyme called CYP3A4 as one way to break this medicine down. Lab studies suggest evodia may slow that enzyme. If it does, more imipramine could linger in your body, which can nudge up the usual tricyclic side effects: dry mouth, constipation, blurry vision, dizziness when you stand, drowsiness, and a faster or irregular heartbeat.

That said, this is only seen in test tubes and animals, not in real people, so I wouldn't lose sleep over it. Imipramine is a sensitive medicine, though, so before adding evodia, please check with your pharmacist or doctor. Don't change your imipramine on your own.

What Is Imipramine Used For?

Brand names: Tofranil, Tofranil PM

Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.

Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
