Ivosidenib falls under QT INTERVAL-PROLONGING DRUGS — the group Evodia interacts with.

Theoretically, evodia might have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias.
Evodia has demonstrated dose-dependent activity as a proarrhythmic agent in animal and in vitro studies. Evodia infusion in animals extends the action duration potential and induces prolongation of the QT interval and Torsade de pointes.

Here's the concern with pairing Evodia and ivosidenib (Tibsovo). Ivosidenib is already known to affect your heart's electrical timing, something doctors call QT prolongation. In lab and animal studies, Evodia has shown a similar effect, lengthening that same timing and even triggering dangerous heart rhythms. So the worry is that stacking the two could add up and raise the risk of an irregular heartbeat (the serious one is called Torsade de pointes).

I'll be honest: this is a theoretical caution based only on test-tube and animal research, not human studies, so the real risk is uncertain. But because ivosidenib already carries a QT warning, I'd play it safe. Please check with your oncology team or pharmacist before adding Evodia.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the gist for your leukemia or bile duct cancer medicine, Ivosidenib (Tibsovo). Your body clears this drug using a liver enzyme called CYP3A4. In test tube studies, evodia appears to slow that enzyme down. If that same thing happened in people, ivosidenib could build up in your blood and lead to stronger effects, including more side effects.

With ivosidenib, one effect I'd watch closely is its impact on heart rhythm (a longer QT interval), along with nausea, fatigue, and a serious reaction called differentiation syndrome. So even a small change in drug levels is worth taking seriously. That said, this is only lab data so far and has not been shown in humans. Please don't start or stop anything on your own. Run it by your oncologist or pharmacist first.

Theoretically, taking CYP3A4 inducers might decrease the levels and clinical effects of evodia.
Animal research shows that concomitant administration of dexamethasone, a known CYP3A4 inducer, with the alkaloid constituents of evodia significantly reduces the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the interesting twist with this pairing: the interaction we have on file runs in the opposite direction from what people usually worry about. Ivosidenib (Tibsovo) can act as a CYP3A4 inducer, meaning it can speed up that liver enzyme system. The concern isn't that your leukemia or bile duct cancer treatment gets weaker, it's that Ivosidenib could lower the levels of Evodia's active compounds, so the supplement may simply not do much.

I'll be honest, this is based only on lab and animal research, not studies in people, so it's quite theoretical. There's no clear danger here, but Evodia isn't well studied and could affect drug-clearing enzymes itself. Since Ivosidenib is a serious cancer medicine, please loop in your oncologist or pharmacist before adding any supplement.

About Ivosidenib

What Is Ivosidenib Used For?

Brand name: Tibsovo

Tibsovo (ivosidenib) is used to treat adults who have a susceptible IDH1 mutation, confirmed by an FDA-approved test. It treats newly diagnosed acute myeloid leukemia (AML) in adults 75 or older, or those who cannot tolerate intensive chemotherapy — either alone or combined with a medicine called azacitidine. It also treats relapsed or refractory AML (AML that has come back or stopped responding to treatment) and relapsed or refractory myelodysplastic syndromes (MDS).

Tibsovo is also approved for adults with previously treated, locally advanced or metastatic cholangiocarcinoma (bile duct cancer that has spread) who carry an IDH1 mutation.
