Here is the interaction we found between these two, with the evidence behind it.
THEOPHYLLINE
Why this applies to Ephedrine, Hydroxyzine, Theophylline
Ephedrine, Hydroxyzine, Theophylline falls under THEOPHYLLINE — the group Fenugreek interacts with.
Animal research shows that fenugreek 50 grams daily for 7 days reduces the maximum serum concentration (Cmax) of theophylline by 28% and the area under the plasma drug concentration-time curve (AUC) by 22%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Your combination product contains theophylline, which helps open the airways and keep your breathing easier. The concern here is with the theophylline piece. Fenugreek, when taken in concentrated supplement amounts, may slightly lower how much theophylline ends up in your bloodstream. If those levels drop, the medicine could work less well, and you might notice more wheezing, chest tightness, or shortness of breath.
I want to be honest about how soft this evidence is. It comes from animal research using a large dose of fenugreek, not human studies, so it's mostly theoretical. A pinch of fenugreek as a cooking spice is very unlikely to matter. Before starting a fenugreek supplement, just check with your pharmacist or doctor so they can keep an eye on your breathing.

“For anyone taking Marax (ephedrine, hydroxyzine, theophylline), nothing on this page matters more than caffeine.
