Antihypertensive Drugs
Why this applies to Sacubitril, Valsartan
Sacubitril, Valsartan falls under Antihypertensive Drugs — the group Fenugreek interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: Entresto (sacubitril/valsartan) already works hard to relax your blood vessels and lower your blood pressure. Fenugreek, when taken in concentrated supplement doses, may nudge blood pressure down a little further. Put the two together and those effects can stack up. This is rated a minor interaction, and the evidence is on the weaker side (much of it comes from animal studies), so for most people it isn't a big worry.
Still, if your blood pressure drops too low you might feel dizzy, lightheaded, unusually tired, or faint when you stand up. The fenugreek you sprinkle on food is very unlikely to cause trouble. A daily supplement dose is the part worth watching. If you'd like to take it, check with your pharmacist or doctor first and keep an eye on your blood pressure at home.

From our pharmacist’s overview on the Sacubitril, Valsartan page
“Entresto (sacubitril and valsartan) is a heart failure medicine, and heart failure patients tend to be heavy supplement users, reaching for fish oil, potassium, and herbal blood-pressure remedies. That instinct carries real risk here.”

About Fenugreek
What is Fenugreek?
- Blood sugar control
- Boosting breast milk supply
- High cholesterol
- Digestive upset
- Low testosterone or libido
Fenugreek is a common kitchen spice that is also taken as a supplement, mainly for blood sugar, cholesterol, and to support breast milk production. Some early research is encouraging for blood sugar, but overall the evidence is mixed and far from settled. It is generally well tolerated as food, but talk with your pharmacist or doctor before using it as a supplement, especially if you are pregnant.
About Valsartan and Sacubitril
What Is Valsartan and Sacubitril Used For?
Brand name: Entresto
Sacubitril and valsartan is used to reduce the risk of cardiovascular death and hospitalization for heart failure in adult patients with chronic heart failure. The benefit is most clearly seen in patients whose heart pumps with a lower-than-normal ejection fraction (the percentage of blood the heart pushes out with each beat).
It is also approved to treat symptomatic heart failure with reduced left ventricular function in children aged 1 year and older. In pediatric patients, it lowers a blood marker of heart stress (NT-proBNP) and is expected to improve heart outcomes.
