Why this applies to Acalabrutinib

Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the deal with these two together. Your body relies on an enzyme called CYP3A4 to clear acalabrutinib out of your system. Goldenseal appears to block that enzyme, so the drug can hang around longer than intended and build up in your blood. Acalabrutinib is not a prodrug, so more drug simply means a stronger effect.

In real terms, that could mean a higher chance of side effects like bruising or bleeding, headaches, fatigue, infections, or an irregular heartbeat. The evidence here comes from human studies (not just lab work), so it's worth taking seriously. Food-level amounts aren't really a concern, but concentrated goldenseal capsules are. Please check with your oncology team or pharmacist before combining them.

P-GLYCOPROTEIN SUBSTRATES

Acalabrutinib also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, goldenseal might increase or decrease serum levels of P-glycoprotein (P-gp) substrates.
There is conflicting evidence about the effect of goldenseal on P-gp. In vitro research suggests that berberine, a constituent of goldenseal, modestly inhibits P-gp efflux. Other evidence suggests that berberine induces P-gp. In healthy volunteers, goldenseal modestly increases peak levels of the P-gp substrate digoxin by about 14%. However, it does not seem to affect other pharmacokinetic parameters such as area under the curve (AUC). This suggests that goldenseal is not a potent inhibitor of P-gp-mediated drug efflux. Until more is known, goldenseal should be used cautiously with P-gp substrates.

Here's the gist for your Calquence (acalabrutinib): it's carried around by a transporter called P-glycoprotein, and goldenseal may nudge that transporter a little. The research is genuinely mixed. Some lab work hints that goldenseal's berberine slightly blocks this pump, while other studies suggest it speeds it up. In real people, goldenseal bumped levels of a similar drug (digoxin) by only about 14%, so it doesn't look like a strong effect.

If goldenseal blocked the pump even a bit, acalabrutinib could climb slightly, which might mean more bruising, bleeding, headaches, or infection risk. The honest answer is we just don't know the exact direction here. Since this is a cancer therapy, please run any goldenseal use past your oncologist or pharmacist before starting it.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
