This Coricidin product is a combo, and the piece that matters here is the dextromethorphan (the cough suppressant). Your body relies on an enzyme called CYP2D6 to clear it. Goldenseal can slow that enzyme down, so dextromethorphan may build up higher than usual.

In real life, that can mean more dizziness, drowsiness, or feeling foggy, and at higher levels even a jittery or off-balance feeling. The chlorpheniramine in this product is already sedating, so the sleepiness could stack up. The acetaminophen part isn't really affected. This is rated a probable, moderate interaction based on modest human evidence. Nothing to panic over, but it's worth a quick chat with your pharmacist or doctor before combining them.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

This Coricidin product is a combo of three medicines, and one of them, dextromethorphan (the cough suppressant), is partly cleared by a liver enzyme called CYP3A4. Goldenseal tends to slow that enzyme down. When the enzyme is blocked, dextromethorphan can build up higher than usual, which may bump up its side effects like dizziness, drowsiness, or feeling spacey or confused. The chlorpheniramine in this product can already make you sleepy, so the two together could leave you extra groggy.

The evidence here is real but modest, and not everyone will notice anything. Concentrated goldenseal capsules matter more than tiny amounts. Before pairing them, please check with your pharmacist or doctor first.

In vitro research shows that goldenseal root extract can inhibit the activity of CYP2E1. However, this effect has not been reported in humans.

Of the three ingredients in Coricidin II, the one this interaction really touches is the acetaminophen (Tylenol). A small share of acetaminophen gets handled by a liver enzyme called CYP2E1. In lab (test tube) studies, goldenseal extract slowed that enzyme down. In theory, blocking it could let acetaminophen linger a bit longer in your system.

Here is the honest part: this has only been seen in vitro, never confirmed in real people, so I would not lose sleep over it. The bigger everyday point is simply not to double up on acetaminophen from several cold products at once. If you take goldenseal regularly or have any liver concerns, run it past your pharmacist or doctor first so they can look at your full picture.

Here's the gist with this combo cold medicine, which contains dextromethorphan (the cough suppressant). Goldenseal carries berberine, and a small human study suggests berberine can slow down a liver enzyme called CYP2D6. That's the same enzyme that helps your body clear dextromethorphan. So in theory, pairing the two could let dextromethorphan build up a bit more than usual.

If that happens, you might notice more of its typical effects, like drowsiness, dizziness, or feeling a little foggy or unsteady. Keep in mind this is theoretical, based on how the drug is processed rather than strong real-world reports, so it's not a reason to panic. Still, since this product also has chlorpheniramine (already sedating), I'd run it by your pharmacist or doctor before combining them.
