Goldenseal interacts with whole categories of medication. Acetaminophen, Chlorpheniramine, Phenylephrine, Salicylamide falls into 2 of those categories, so it can interact with Goldenseal in 2 different ways. CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Chlorpheniramine, Phenylephrine, Salicylamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

This combination cold-and-allergy product mixes a few ingredients, but the one most likely to feel the effect of goldenseal is the chlorpheniramine, the antihistamine. Goldenseal tends to slow down the CYP3A4 enzyme that helps clear some of these ingredients, so they may linger a bit longer in your body. In plain terms, you could feel more of the usual effects, like drowsiness, dry mouth, or grogginess from the antihistamine.
The good news is that this is rated a moderate, only possible interaction, and the evidence is modest. Occasional use is unlikely to cause big problems for most people. Still, if you take goldenseal regularly or feel extra sleepy or jittery, check in with your pharmacist or doctor before combining them.

CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES
2 of 2Acetaminophen, Chlorpheniramine, Phenylephrine, Salicylamide also falls under CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES.
In vitro research shows that goldenseal root extract can inhibit the activity of CYP2E1. However, this effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo product (which includes acetaminophen, also called Tylenol) is the one piece here that involves the liver enzyme CYP2E1. Lab studies hint that goldenseal might slow this enzyme down. Since CYP2E1 is one pathway that breaks acetaminophen down, blocking it could, in theory, let a bit more of the drug hang around. Acetaminophen is generally well tolerated, but the main concern with higher exposure is stress on the liver.
Here's the honest part: this has only been seen in test-tube studies, never confirmed in real people, so I wouldn't lose sleep over it. The bigger, well-known rule still applies, just don't exceed the acetaminophen dose on the label and avoid heavy alcohol. If you take goldenseal regularly, mention it to your pharmacist so they can check your total daily acetaminophen.
