Goldenseal contains berberine. Animal research shows that berberine can have sedative effects. However, this effect has not been reported in humans.

This combination pill already has two ingredients that can make you sleepy: codeine, an opioid pain reliever, and chlorzoxazone, a muscle relaxant. The concern here is that goldenseal contains berberine, which showed mild sedative effects in animal studies. In theory, stacking it on top of these medicines could add to the drowsiness, leaving you more tired, foggy, dizzy, or unsteady on your feet.

Honestly, this is a theoretical worry. The sedative effect has not actually been seen in people, so I wouldn't lose sleep over it. Still, if you notice extra grogginess, go easy on driving and let your pharmacist or doctor know before adding goldenseal.

This combo pill is interesting because the codeine part behaves differently than you might expect. Codeine is actually a prodrug, meaning your body has to switch it on using an enzyme called CYP2D6 to turn it into morphine, which is what actually relieves pain. Goldenseal blocks that same enzyme. So instead of building up, your codeine may convert to less active painkiller, leaving you with weaker pain relief while still getting side effects like drowsiness, constipation, or nausea.

The evidence here is moderate and likely real, though not airtight. Concentrated goldenseal supplements matter far more than trace amounts. Before mixing them, please check with your pharmacist or doctor, especially if your pain isn't well controlled.

This combination pill pairs the painkiller acetaminophen, the muscle relaxant chlorzoxazone, and the opioid codeine. The piece that touches the goldenseal question is chlorzoxazone, which the body clears using a liver enzyme called CYP2E1. In test-tube studies, goldenseal seems to slow that enzyme down. If that held true in people, chlorzoxazone could build up a bit and leave you feeling more drowsy or dizzy than usual.

Here is the reassuring part: this has only been seen in the lab, not in real patients, so it is still theoretical. I wouldn't lose sleep over it, but since codeine already adds its own sleepiness, it's worth a quick chat with your pharmacist before combining goldenseal with this medication.
