NyQuil already packs a sleepy punch, mostly from the doxylamine (an antihistamine) and partly from dextromethorphan. The concern here is simple: goldenseal contains berberine, and in animal studies berberine showed some sedative effects. So in theory, layering goldenseal on top of NyQuil could leave you feeling more drowsy, groggy, or foggy than usual.

I want to be honest, though: this sedation effect has only been seen in animals, not in people, so it's a theoretical caution rather than a proven problem. Still, if you're driving or doing anything that needs focus, play it safe. Before combining them regularly, have a quick chat with your pharmacist or doctor.

Theoretically, goldenseal might increase serum levels of dextromethorphan.
Goldenseal contains berberine. A small clinical study shows that berberine can inhibit cytochrome P450 2D6 (CYP2D6) activity and reduce the metabolism of dextromethorphan.

Here's the deal with NyQuil and goldenseal. The cough-suppressing ingredient, dextromethorphan, is cleared from your body mainly by a liver enzyme called CYP2D6. Goldenseal contains berberine, which can slow that same enzyme down. So in theory, the dextromethorphan in your dose may not be broken down as quickly and could build up a bit more than usual.

If that happens, you might feel more of dextromethorphan's effects, like drowsiness, dizziness, or feeling foggy, which can add to the sleepiness doxylamine already causes in NyQuil. I want to be honest though: this is based on pharmacology and a small study, not strong proof. It's worth mentioning to your pharmacist or doctor before combining them, especially if you'll be driving.
